The information you provide in this form will be used for the the Glimmerglass website artist bios, as well as other publicity/marketing purposes such as news stories and social media. This will NOT be used for the Program Book.
Please make sure to upload your headshot to the PIF (Personal Information Form) on the Company Portal homepage. That is the only Glimmerglass form where you will be asked for your headshot.
